Please have your insurance information available when making your appointment. If you do not have insurance, or your plan is not listed here, payment will be due in full on the date of service. If you expect to be reimbursed for your services by your insurance company we are happy to provide a detailed receipt.

If your insurance requires a referral please make sure that you obtain one prior to coming for your appointment. If you have not obtained a referral in advance you will be responsible for the full cost of the examination.

As a courtesy to our patients, we will file insurance claims for all insurance plans in which we participate. Please keep in mind that you are responsible for payment above and beyond what your insurance covers including, but not limited to, co-payment, co-insurance and/or deductible. Any charges not covered by your insurance are due at the time of service, or upon receipt of a statement from our office after insurance has processed your claimwhere any reimbursement from your insurance carrier will be reflected.
